2. You then must choose one of the following agreements :

   then print documents from your computer.
3. Read through materials.
4. Fill out all documents listed on Greetings page. Then send all required documentation with a one time application processing fee of $10.00 by either :
   
   • US Mail, or
   • Scanning the printed copies, then e-mailing them to daps@daps-inc. com.
   • You may also telephone us to set up an appointment to complete the application in our office.
   • PLEASE DO NOT FAX YOUR DOCUMENTS AS THEY ARE VERY DIFFICULT TO READ.
